Etiologic Subtypes, Risk Factors And Outcome Of Acute Ischemic Stroke With Young Patients

Abstract
INTRODUCTION: Stroke in people under the age of 45 years old of age is less frequent than in older populations but has a major impact on the individual and society. The aim of this study was to determine the etiologic subtypes of acute ischemic stroke with young patients.
METHODS: We reviewed the charts of the 619 patients who were admitted with acute ischemic stroke between the dates January 2011 and November 2014. An acute ischemic stroke with young patients was defined as age of 45 and under the age of 45 years old. The demographic data, the National Institutes of Health Stroke Scale (NIHSS) scores at admission and detailed investigations to determine etiologic focus were recorded. We determined etiologic stroke subtypes using the automated Causative Classification System (CCS). The modified Rankin Scale (mRS) scores were recorded in follow up period.
RESULTS: There was 32 (5.2%) young patients with acute ischemic stroke. Hypertansion, DM, AF and CAD was significantly lower in young patients compered to older than 45 years old patients (p<0.05). The mean NIHSS score at admission and mortality in hospital was significantly lower in age of 45 and under the age of 45 years old patients compared to older than 45 years old patients (p=0.006, p=0.043). The cardioaortic embolism was the most common etiologic stroke subtype in both groups. Only other causes was significantly higher in young acute ischemic stroke group compared to older than 45 years old patients group. The median mRS in follow up period was significantly lower in age of 45 and under the age of 45 years old patients compared to older than 45 years old patients (p<0.001).
DISCUSSION AND CONCLUSION: Young patients with ischemic stroke showed different risk factors, stroke etiology, stroke severity and prognosis compared to older than 45 years old patients with ischemic stroke.
