Abstract

Background: Churg-Strauss syndrome (CSS) characterised by asthma, sinusitis, hypereosinophilia and peripheral neuropathy is a systemic necrotising vasculitis affecting small and medium sized vessels. Peripheral neuropathy is the most common neurological feature. Objective: To evaluate clinical and laboratory findings and treatment regimenin CSS which is an unusual vasculitis. Findings: 38 years old female patient with burning, parestesia and weakness at right food for two months. Hypertension, sinusitis and asthma were also present at background. In neurological examination, loosing of deep tendon reflexes and 1/5 power and sensation in the peroneal and tibial nerve area were found (anaesthasia-analgesia) on the right lower limb. Eosinophilia and increase of lgE were determined. EMG was revealed as diffuse neurogenic pattern and total lesion was found on the tibial and peroneal nerve. Demyelination and lymphocytic- eosinophilic vascular infiltration with axonal degeneration were found on sural nerve biopsy. Oral prednisolon and IVIG were used in the treatment. Clinical and laboratory findings were improved during the posttreatment follow up. Conclusion: CSS and other necrotising vasculitis should be thought in the patients with peripheral neuropathy as one of the differential diagnosis.
