Abstract

We report a patient presenting with kinetic tremor and cerebellar atrophy was diagnosed to have cerebellar degeneration. Search for an occult malignancy revealed lg A? type multiple myeloma (MM) and the patient was considered as paraneoplastic cerebellar degeneration (PCD) due to MM. In our case, immunophenotype of MM, localization and progression of symptoms were different from the first case in the literature reported as PCD due to MM. Monoclonal lg A? immunoglobulin was detected in cerebrospinal fluid (CSF) by immunofixation where CSF protein electrophoresis was normal. We considered the role of CSF M protein in PCD pathogenesis.
