Carotid-ophthalmic ( or paraclinoid ) aneurysms which arise from the C2 and distal C3 portions of the internal carotid artery are rarely reported in the literature. Initial presenting symptoms are either subarachnoid hemorrhage or progressive visual loss due to the optic nerve compression. A female patient with progressive visual loss and periorbital pain in her left eye is presented with her CT and angiography finding demonstrating unruptured large carotid-ophthalmic segment aneurysm.

Keywords: Carotid-ophthalmic aneurysm, Paraclinoid region, Progressive visual loss.
