Initial presenting symptoms are either subarachnoid hemorrhage or progressive visual loss due to the optic nerve compression. A female patient with progressive visual loss and periorbital pain in her left eye is presented with her CT and angiography finding demonstrating unruptured large carotid-ophthalmic segment aneurysm. Keywords: Carotid-ophthalmic aneurysm, Paraclinoid region, Progressive visual loss.
