Spontaneous bilateral intracerebral hemorrhages are extremely rare. To further characterize this rare event, we report a 60-year-old man with chronic hypertension, who presented with bilateral subthalamic hemorrhages that consequently resulted in monoplegia in the right upper extremity and loss of consciousness.
