A Case of Neurosarcoidosis With Atypical MRI Findings

We report a 36-year-old female presenting with ocular, pyramidal, sensory and cerebellar signs who was diagnosed as neurosarcoidosis. She had no complaints or clinical signs but radiologic findings consistent with lung involvement. Her MRI study revealed multiple homogenous nodular lesions, localised on the leptomeningeal surfaces, in the periventricular and subependymal areas and intraparenchymally, which became only visible with administration of intravenous Gadolinium-DTPA. This finding is unusual and has not been reported as one of the typical MRG findings in neurosarcoidosis. We want to point out the importance of administering Gadolinium-DTPA to all cases with suspected neurosarcoidosis in order to show lesions otherwise invisible.
