Hot Cross Bun” Sign in a Patient With Multiple System Atrophy and REM-Behaviour Disorder

Scientific BACKGROUND: Multiple system atrophy (MSA) is a sporadic adult-onset neurodegenerative disease characterized by progressive cerebellar ataxia, basal ganglia symptoms and autonomic failure. MSA-C is a subtype of MSA, which indicates the predominance of cerebellar features.
CASE: We here present a 70-year-old male patient diagnosed as MSA and REM-behaviour disorder whose cranial magnetic resonance imaging (MRI) showed “hot cross bun” sign. CONCLUSION: In addition to clinical symptoms, “hot cross bun” sign observed in MRI indicating the presence of degeneration in pons is a very useful finding in the diagnosis of MSA.
