Albeit intravenous tissue plasminogen activator (tPA) is one of the key elements of acute ischemic stroke management, its use has not become widespread as it should be. One of the major reasons for this apparent under-utilization is being left unanswered of many frequently asked questions about its use. In this part of the review, the questions about systemic non-neurological conditions affecting clinical decision making and management of tPA-related complications, collected from the neurologists practicing in Turkey, were answered on the basis of current literature, and clear recommendations were provided.
